Worksheet for Function Based Behavior Support Planning
The Worksheet for Function-Based Thinking is an individualized approach to addressing a student's concerning behavior. It may be a step to try before conducting a Functional Behavior Assessment.

USE A TEAM APPROACH:
- people knowledgeable about the student - people who see the student in multiple settings, when appropriate (such as gen ed and sped) - a person who is familiar with FBA and/or has training in using the worksheet. ATTACH DATA COLLECTION SHEETS: Tracking data is essential to determining if the intervention is working. If the student's behavior is not changing, we use data to determine if we are implementing the intervention correctly and consistently. REVIEW THE PLAN: Best practice suggests the plan should be reviewed at least every 6-8 weeks. Teams should use the Review & Next Steps document to review data on behavior change and determine implementation fidelity. For an 80 minute online training on
